Western Australian Home Care (HACC)

The aim of the WA program is to help people who are frail or have a disability to live independently in the community by supporting them to do so. 

The program is underpinned by the “Wellness” philosophy and encompasses physical, social and emotional aspects so that people they can live autonomously.  In summary HACC:

  • Addresses your needs in a holistic way 
  • Enables you to set their own goals and make decisions about the support they receive 
  • Ensures support is delivered in partnership with you 
  • Encourages you to remain involved in your community and maintain social connections 
  • Supports your choice and decision-making


  • Face to face assessments are done by a HACC Assessment Service, which is separate to My Aged Care.
