'Extended Aged Care at Home' Packages

Extended Aged Care at Home (EACH) packages are flexible, individually designed packages funded by the Commonwealth Government for frail older Australians who require a high-level care to remain living at home.  

This would equate to a Home Care Package Level 4 which is offered in states and territories outside Western Australia.  To be eligible you must be assessed by an ACAT and determined as requiring a high level of care.

The Service Provider will tailor services to the individual and coordinate care services.  Services might include:

  • Registered nursing care
  • Allied healthcare
  • Personal care
  • Transport to appointments
  • Social support
  • Home help
  • assistance with oxyfen and/ or enteral feeding

EACH packages are coordinated and administered in the same manner as CACPs.

Extended Aged Care at Home Dementia Packages

Extended Aged Care at Home Dementia Packages (EACHD) are packages of care similar to EACH packages with the same range of services provided, however they are tailored to help older people who are experiencing behavioural and psychological difficulties associated with dementia.
